Event Description
A physician reported that apparently the 23 gauge cannula appeared to be sticky while removing the trocar blade.All three required the grasping of the cannula hub with 12 forceps to retain in place otherwise it was clear the entire trocar cannula connection would pull itself back out of the eye.The product was used to complete the procedure and there was no patient harm.Sample was not available.Additional information could not be performed as the customer was unwilling/unable to provide additional information.This is one of two reports.
 
Manufacturer Narrative
A sample was not received at the manufacturing site for evaluation; therefore, the condition of the product could not be verified.A review of the device history record traceable to the reported lot number, indicates that the reported product was processed and released according to the reported product¿s acceptance criteria.The photo provided was reviewed by the manufacturing site.The photo is of a pak label, the reported product and lot information is confirmed.The video attached to the parent complaint was reviewed by the manufacturing site.The video is of an eye surgery in progress, the reported product complaint could not be confirmed.A sample was not received at the manufacturing site and the device history record review of the lot number provided indicated the product was processed and released according to the product¿s acceptable criteria, therefore; the root cause for the customer complaint issue cannot be determined.The exact root cause for this complaint is unknown, therefore, specific action with regards to this complaint cannot be taken.All trocar assemblies are 100% inspected for gage size.Any non-conformances found are removed from the lot and scrapped.Complaints are reviewed and monitored at regular intervals for any significant adverse trends.No adverse trends have been observed associated with the reported product and event.No further actions are required.The manufacturer internal reference number is: (b)(4).
