The device was not received for evaluation; however, the device was evaluated on site by a qualified technician.Upon evaluation it was observed the base of the molding that holds the wheel was cracked and the wheel shaft was broke off.Photographs of the sample were also provided for evaluation.Visual inspection of the pictures showed the wheel disconnected from the foot unit. The reported condition was verified. The possible cause of the condition could be use related as it appeared the unit was run into something at a high-speed causing the crack and break.A service history review revealed no indication that the parts replaced during servicing caused or contributed to the reported event.Should additional relevant information become available, a supplemental report will be submitted.
