A visual inspection was performed on the returned device.The reported tip break was unable to be confirmed.The reported stretched tip was confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents reported from this lot.The investigation determined the reported break and stretched tip appear to be related to case circumstances of the procedure.It is likely that during preparation, the tip of the barewire was handled causing the coils to stretch at the step causing the appearance of a break.There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
