Event or Problem Description
Information was received from a healthcare provider (hcp) via manufacturer representative regarding a patient who underwent unknown spinal therapy for idiopathic scoliosis.It was reported that when attempting to remove the r2 break-off set screw, the t25 obturator was broken.The counter was used, but the tip of the device remained inside the set screw with the screw broken off.Suction was used and the remaining parts were removed.No further complications or symptoms were reported.Additional information was received via manufacturer representative that the lot number of the reported product is (b)(4).There were no fragments left inside the patient.
