Device report from synthes reports an event in (b)(6) as follows: it was reported on (b)(6) 2022, that during an unknown procedure, it was observed that the insertion arc of the multiloc humeral nailing (mhn) system at the moment of inserting the nail, began to loosen and the surgeon needed to rearrange the nail.During the removal of the device to rearrange the nail, it was observed that the connection with the insertion arc was not gripping well, and the top of the nail raised a fragment. The surgeon fixed that part and introduced the nail again; however, it continued to fail and interfered with the proximal locking since it did not match. An option was given of changing the nail for another, but the surgeon preferred to continue. When performing the freehand distal locking, the surgeon requested a 2.5 drill bit with a smaller diameter and at the time of impacting, it bended, and the 3.2 drill bit was passed, which was the recommended one, and at the time of impacting it and after drilling it also it bended. There was a surgical delay to the procedure and the surgery was completed successfully. No fragments were generated. No further information is available. This report involves one (1) insert-handle f/multiloc hum nail syst. This is report 1 of 1 for complaint (b)(4).
