Additional Manufacturer Narrative
On may 26, 2022, device evaluation was completed.Device evaluation summary: mentor conducted visual inspection and microscopic examination of the returned device.Visual analysis of the returned sample determined that the sx mpp gel 225cc breast implant was found to have a tear on the anterior view, measuring approximately 2.8 cm (tear a).In addition, a second tear was observed on the posterior view, measuring approximately 2.0 cm (tear b).Also, an additional area of missing material was observed on the anterior view, measuring approximately 2.0 cm x 1.5 cm.Microscopic examination was performed on the edges of the all ruptures, and the cause of the tear b could not be identified, and parallel striations were found in an area of the tear a and missing material, measuring approximately 0.5 cm (tear a) and 0.3 cm (area of missing material).Parallel striations are consistent with markings made by a sharp object perforating the implant shell. The cause of the rupture in the remaining area of the tear a and missing material could not be identified.A manufacturing record evaluation was performed for the finished device lot number, and no non-conformances were identified.Although no conclusion could be reach on the cause of the rupture b, the instructions for use contain the following caution: rupture can occur at any time after implantation but is more likely to occur the longer the implant is implanted. The following may cause implant to rupture: stressing the implant during implantation and weakening it; folding or wrinkling of the implant shell; excessive force to the chest (e.G.During closed capsulotomy); trauma; compression during mammographic imaging; and severe capsular contracture.Breast implants may also simply wear out over time.Based on the date of implantation provided and the expiration date of the reported lot number, the device was implanted after the expiration date. Please note that according to the product insert data sheet, sterility cannot be guaranteed if the packaging has been damaged or after the ¿use by date¿ shown in the box label. Therefore, the device was used in an off-label manner. As part of mentor¿s quality process, all devices are manufactured, inspected, and released to approved specifications.Additional complaint information monitoring for potential safety signals is conducted through complaint trending as part of post market surveillance.Manufacturer¿s reference number: (b)(4).
