The following fields were updated due to additional information: d9: device available for eval yes, d9: returned to manufacturer on: 31-may-2022.H6: investigation summary: customer returned (19) used bd pen needles without tear drop labels, outer covers, or inner shields attached. The customer reported that the needles break off in the injection site, and the needles bend at the patient end during injection. The returned samples were examined, and it was observed that all 19 pen needles featured a broken patient end (pe) cannula.Microscopic examination of the samples revealed characteristics such as residual bends on the hub-end of the fractured cannula, and ovality (deformation from the normally circular cross section) ¿ removal of the epoxy made this evident. When viewed together these are all indicators of bending/re-straightening mode of failure.A lot history review was carried out and no related non conformances were raised in association with this packaged lot concluding all inspections were performed as per the applicable operations and met qc specifications.Embecta was able to duplicate or confirm the customer¿s indicated failure (pe cannula broken) the root cause for this issue is user related. The pe cannula was broken by the user after handling the pen needles.H3 other text : see h10.
