Event Description
|
Physician used a chocolate pta balloon during treatment of a calcified lesion in the patient's proximal posterior tibial artery.
moderate vessel tortuosity and calcification are reported.
Lesion exhibited 60% stenosis.
There was no damage noted to packaging, i.

Shelf carton, hoop/tray.
There were no issues noted when removing the device from the hoop/tray.
The device was prepped as per ifu without issue.
The device was not passed through a previously deployed stent.
No resistance was encountered during advancement.
It is reported a break/fracture occurred on the tip of the device during advancement through the guide catheter.
The detached tip was removed with the catheter.
The procedure was completed with a different brand device.
No patient injury reported.
