Block h2: additional information: block d4 (lot number) and h4 (device manufacture date) block h6: device code a0402 captures the reportable event of balloon burst.Block h10: investigation result a visual examination of the returned complaint device revealed the balloon was torn longitudinally. No kinks or damage was found on the catheter of the device. The balloon was not folded which indicates that the device was subjected to positive pressure. An examination of the markerbands and the tip of the device identified no problems. It is possible that the device may have encountered difficult patient anatomy such as stenosis, calcification or tortuosity, which could have contributed to the longitudinal tear in the balloon. It should also be considered that based on the event description, that the device may have been inflated above the rated burst pressure of 20 atm causing the balloon to rupture. Therefore, the most probable root cause is adverse event related to procedure.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.A review of the instructions for use (ifu) and product labeling was completed and did not reveal any evidence of device misuse or that the device was used in a manner inconsistent with the labeled indications.
