It was reported that an one-link non-dehp standard bore catheter extension set that was attached to an unspecified intravenous (iv) catheter was cracked and squirting out of the tube.This was observed after an iv start, after the tubing was primed and the nurse started flushing with normal saline.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
