Event or Problem Description

Medtronic received a report that the mirage guidewire tip broke. The patient was undergoing treatment for an intracranial arteriovenous malformation (avm). The patient's vessel tortuosity was normal. The access vessel was the right femoral artery, which was 8mm in diameter. It was reported that mirage guidewire was placed in the blood vessels and rotated for about 2 turns before the tip of the guidewire broke and escaped.A solitaire ab was used to capture and successfully remove the broken guidewire segment. the surgeon was asked to estimate the length of the broken tip, and it was said that they thought it to be the short part of the tip.The surgeon noted that in the same surgical procedure and position multiple guidewires had been rotated many times but this breakage had not occurred. the device was replaced, and the patient did not experience any injury, symptoms, or complications. The devices were prepared and flushed according to the instructions for use (ifu).Ancillary devices include a marathon catheter and a rebar catheter.
