Medtronic received information regarding a navigation system being used intraoperatively during a sacroiliac and thoracolumbar procedure.It was reported that there was an issue with the driver instrument.The site had a second driver instrument that was used to complete the case.There was no surgical delay and no impact on the patient outcome.Additional information was provided stating the driver had become disengaged at the screw/driver interface and when the surgeon went to do the final tightening on the screw, the tip of the driver came in contact with dense bone and snapped.The surgeon then used a non-navigation screwdriver which has a locking mechanism to prevent the screw from becoming disengaged from the driver interface.The surgeon suggested that the navigated drivers should have that same locking mechanism to prevent loosening of the screw.
