An attempt was made to use a resolute onyx coronary drug eluting stent to treat a mildly tortuous, severely calcified lesion with 99% stenosis in the ostium of the proximal right coronary artery (rca). The device was inspected with no issues noted. It was reported that negative prep was performed with issues noted. The negative prep was then performed during the procedure. The lesion was pre-dilated.The device did not pass through a previously deployed stent.Resistance was encountered when advancing the device. It was reported that stent deformation occurred in vivo during positioning/advancement.When the stent deformation was identified an attempt was made to remove the whole system however it was reported that stent dislodgement occurred during the removal following failed delivery. The dislodged stent was lost in the patients arm pit.The stent was left in the patients ulnar artery, and the patient appeared to be doing fine. The procedure was completed by implanting a resolute onyx 2.0 x 26mm device in the right coronary artery (rca). The patient is reported as being alive with no injury.

Additional information: two still procedural images were provided for review. The first procedural image is of the stent in vasculature and appears to show the attempted delivery of the stent. The stent and delivery system are positioned just distal to the tip of the guide catheter.Although the mechanism of delivery cannot be confirmed from a still image this image suggests that stent may have been damaged during unsuccessful delivery resulting in the stent struts being damaged and raised. The raised stent struts most likely impacted within the guide catheter during removal attempts resulting in the dislodgement in the ulnar artery. The second image confirms the presence of a dislodged stent in the ulnar artery. The mechanism of dislodgment is not identified from the images medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms. This statement should be included with any information or report disclosed to the public under the freedom of information act.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report. If information is provided in the future, a supplemental report will be issued.

Additional information: the negative prep issues were not noted prior to use in the patient.A negative vacuum was not pulled on the device while the device was in the patient. The stent was left in the patients ulnar artery and no attempts were made to remove the device as the patient appeared to be doing fine.No intervention was required to secure the dislodged stent in the ulnar artery. The stent was not implanted but was left floating. The resolute onyx 2.0 x 26mm device was deployed with no issues.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report.
