Conclusion: overload fractures in the active electrode which are consistent with an external mechanical impact were determined to be the root cause of device failure. In addition a reduced electrical impedance between coil and eap/rg (evoked action potentials/ remote ground) electrode was discovered.Furthermore, it was reported that the recipient suffered from facial nerve stimulation since activation.A contribution of the lower-ohmic connection between coil and reference electrode cannot be excluded nor confirmed. This is a final report.
