Additional Manufacturer Narrative
Device evaluation: the rms-060028-r device of lot number c1879495 involved in this complaint was returned for evaluation, without the original packaging. With the information provided, a physical examination and document-based investigation was conducted.Document review prior to distribution rms-060028-r devices are subjected to a visual inspection and functional checks to ensure device integrity. These inspections and functional checks are outlined in internal procedures in place at cirl.A review of the manufacturing records for rms-060028-r of lot number c1879495 did not reveal any discrepancies that could have contributed to this complaint issue. The review of relevant manufacturing records confirms the failure mode has not previously occurred with the current lot number.Based on the information available to date, there is no evidence to suggest that there are any manufacturing issues associated with lot number c1879495.It should be noted, an nc code was noted on the work order, however this was subsequently reworked and would not have attributed to this complaint issue. It should be noted that the instructions for use states the following: ¿individual variations of interaction between stents and the urinary system are unpredictable¿.There is no evidence to suggest the user did not follow the ifu.Image review: n/a.Root cause review: a definitive root cause could not be determined as the circumstances of use cannot be replicated in the laboratory.A possible root cause could be attributed to the stent failing to maintain patency possibly due to the encrustation which was noted during laboratory evaluation. It should be noted there were significant attempts made to attain clarification regarding when the hydronephrosis occurred. The additional information has been requested.Once received, the file will then be updated accordingly.Per clinical input ¿i would imagine that the hydronephrosis was present before the stent was placed.Hydronephrosis is swelling of the kidney before of a build up of urine because of an obstruction. It sounds like in this case the stent failed to provide patency so the hydronephrosis would have persisted.I note the cause of the obstruction is not noted in the complaint ie.Whether it was due to extrinsic compression from a tumour for example. It is also unusual for stent encrustation to happen after only a short indwell time but not impossible. It could be that the stent encrustation was the cause of the obstruction.¿ summary: the complaint is confirmed based on customer testimony.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.Complaints of this nature will continue to be monitored for potential emerging trends.
