This report is being submitted in pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been able to investigate or verify prior to the required reporting date. This report does not reflect a conclusion by depuy mitek or its employees that the report constitutes an admission that the device, depuy mitek, or its employees caused or contributed to the potential event described in this report. If information is obtained that was not availabe for the initial medwatch, a follow-up medwatch will be filed as appropriate.H10 additional narrative: investigation summary: the product was returned to mitek for evaluation.Mitek then conducted visual inspection of device received.Upon visual inspection, the device showed wear marks, but in expected condition. When inspecting at the handle¿s area, some malleting marks could be observed, the handle¿s cap is broken due to the hard use.The welding near the connection point was broken.A manufacturing record evaluation was performed for the finished device lot number:18d01, and no nonconformances were identified.According with the visual evaluation result, this complaint can be confirmed. The possible root cause can be attributed to the constant and rough use.Since this is a reusable device, the constant manipulation between surgeries and repeated sterilization process can lead to a broken handle's cap; the broken condition can related to a welding issue; when applying excessive force after multiples uses affecting the connection point. However, this cannot be conclusively determined. As per ifu, inspect the instruments before use for integrity and compatibility in order to ensure proper functionality and safety.Prior to sterilization, instruments should be cleaned by either manual or automated means.Care should be taken to avoid the two osteotomes hitting one another as damage to the cutting edges can occur. At this point in time, no corrective action is required, and no further action is warranted. However, depuy synthes mitek will continue to monitor additional complaint information for potential safety signals through complaint trending as part of post market surveillance.
