It was reported that during a cori assisted ukr surgery the real intelligence robotic drill had a "system time out" on a few occasions and the error would pop up during milling. They managed to complete milling of femur.During milling of the tibia, the system crashed, was frozen and unresponsive and they had to restart the system. The procedure was completed, with a non-significant delay, using a s+n back-up device.Patient was not harmed beyond the problem reported.

H3, h6.The real intelligence cori, rob10024, (b)(6)., used in treatment was not returned for evaluation, however there is a video and picture which indicate the drill exposure motor is dead.A relationship between the reported event and the device was established. The drill, ((b)(6).) was returned and the investigation confirmed an exposure motor failure. The log files were provided but are for another device on a different day so they add no value to the investigation. The most likely cause of this event is associated with the drill¿s exposure motor that is not functioning properly.A review of manufacturing records indicate the software met all specifications upon release into distribution.A complaint history review for similar reported/confirmed complaints has identified prior events.A historical escalation event review was completed.A review of prior escalation actions found no actions applicable to the scope of this case. The failure mode and associated risk have been anticipated within the risk file and the documented risk level is still adequate.Although no further containment or corrective action is recommended or required at this time, all complaints are monitored and trended through post market surveillance activities.
