Event Description
|
It was reported that the right ventricular (rv) lead triggered an audible alert.Interrogation revealed an elevated sensing integrity counter (sic) and noise was observed on non-sustained episodes.Device manipulation produced noise and variable impedances as well, and a lead fracture is suspected. The lead was reprogrammed and remains in use, however hospitalization was required. No patient complications have been reported as a result of this event.

Event Description

It was further reported that during a lead revision procedure, the helix would not retract.Even though the operator observed rotation of the lead pin, there was no torque transfer at the fixation point at the helix.A stylet was eventually used, and gentle traction caused the helix to straighten before releasing from the ventricular wall. The lead was fully removed and replaced.
