Event Description
The hospital reported that during a coronary artery bypass procedure using hst iii system (3.8mm).They stated that they unlocked the heartstring cutter and fired it into the aorta in the usual fashion.They stated that "it didn't feel the same".They noted that the aortic core was attached to the barb as designed and that is also when they noticed that the device handle was split at the seam at the level of the lock and button actuator.They was able to complete the procedure without issue.No other device complaints.No delay claimed.No parts falling off claimed.No harm to patient claimed.

(b)(4).The device was returned to the factory for evaluation on 06/03/2022.An investigation was conducted on 12/06/2022.A visual inspection was conducted.Signs of clinical use and no evidence of blood was observed.The aortic cutter was in a deployed state.There were no visual defects observed on the cutter.The housing case was observed to be split in half at the base of the aortic cutter.Based on the returned condition of device as well as the investigation results, the reported failure "break" was confirmed.The certificate of conformance (c of c) was reviewed.The vendor certifies that this device lot conforms to all applicable product specifications.The lot # 25162417 history record review was completed.There were no ncmrs, rework, or deviations documented for the reported lot number.Based on the dhr/lhr review results, it was determined that there is no relation between the batch manufacturing process and the reported failure.Specific actions for the reported/analyzed failure mode are being maintained and documented under maquet's corrective and preventive action (capa) system.
