It is alleged that the patient received a gunther tulip inferior vena cava (ivc) filter on (b)(6) 2008, and the patient was injured without further explanation.Hospital and medical records have been requested, but not yet provided.

Event Description
Patient allegedly received an implant on 21jul2008 via the right femoral vein for prevention of deep vein thrombosis (dvt) and pulmonary embolism (pe).Patient is alleging tilt and vena cava perforation.The patient further alleges "vibrations" in chest and body, as well as pain, discomfort and depression.Report from xray: "the tip of an ivc filter lies at the level of the superior one third of the l2." report from ct (computed tomography): "1.Filter tilt: there is 5° of posterior tilt of the ivc filter apex which is 3 mm from the posterior ivc wall.2:positioning: the apex of the filter is 3 mm above the inferior wall of the right renal vein which is the lowest renal vein.3.Perforation: the 4 filter struts are outside of the ivc walls.There are fat planes between 1 and 3 mm at each.4.Filter strut fracture or bending: none.5.Stenosis of ivc: none.".
