H10 h6 the reported device was not returned to the designated complaint unit for independent evaluation, thus visual inspection and functional testing could not be performed. There was a relationship found between the returned device and the reported event.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specification upon release for distribution.A complaint history review found no similar reported events. The instructions for use was reviewed and found to include conditions of off label use and technique specifics, as well as precautions and warnings related to the use of the device.A risk management review found that the reported failure was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate.Without the requested clinically relevant supporting documentation, the root cause of the reported ¿tip of the shaver popped off¿ cannot be definitively concluded. The patient impact beyond the reported is not anticipated as the broken piece was ¿fully retrieved¿ from the patient using a backup device with a non-significant procedural delay. No patient harm has been alleged. Therefore, no further clinical/medical assessment can be rendered.A review of the customer provided images found the blade pictured with the distal tip of the outer blade sheared off and separated from the rest of the blade assembly. The complaint was confirmed but the root cause could not be determined as the device has not been returned for evaluation.Factors that may have contributed to the reported event include an unintended impact force to the tip of the device or excessive side loading. No containment or corrective actions are recommended at this time.
