Additional Manufacturer Narrative

The device will not be returned for evaluation however the provided photographic evidence exhibits the reported event. The manufacturing documents from the device history record have been reviewed with special attention to the manufacturing and inspection of the product. The product released for distribution was found to have met all specifications prior to shipment. This is the only complaint for this lot number and failure mode within the past two years.A two-year review of complaint history revealed there has been a total of 9 complaints, regarding 9 devices, for this device family and failure mode.During this same time frame (b)(4) devices have been manufactured and shipped worldwide.Should all the complaint devices have been found confirmed for this reported failure, the rate of failure would be (b)(4).Per ifu care must be taken to line up the lobes of the driver with the slots in the screw.Failure to ensure full engagement may result in implant breakage.Examine the driver prior to screw engagement for gouges, scratches or dents.Be sure the tip is not bent so as not to impede the engagement of the screw. The presence of these defects increases the risk of screw breakage.Use only with a straight 0.045 inch diameter guidewire.Use of a larger diameter guidewire will risk breakage of the bioscrew.Do not kink (sharply bend) the guidewire prior to or during insertion.Breakage may occur if the bioscrew is used with a kinked guidewire this issue will continue to be monitored through the complaint system to assure patient safety.
