It was reported the distal end of inserter broke off during implantation of the stem.The broken piece was found in its entirety, and it did not prolong the surgery.No adverse events have been reported as a result of the malfunction.Attempts have been made and additional information on the reported event is unavailable at this time.

(b)(4).Unknown taperloc stem.Customer has indicated that the product will not be returned to zimmer biomet for investigation, due to the device was requested, but not released by the hospital.The investigation is in process.Once the investigation has been completed, a follow-up mdr will be submitted.

This follow-up report is being submitted to relay additional information.Visual examination of the provided pictures identified the tip of the threads is missing a chunk.The shaft shows signs of wear.Review of the device history record(s) identified no deviations or anomalies during manufacturing.A definitive root cause cannot be determined.If any further information is found which would change or alter any conclusions or information, a supplemental report will be filed accordingly.Zimmer biomet will continue to monitor for trends.
