Additional Manufacturer Narrative
H10: manufacturing review: a manufacturing review was not required as this is the only complaint reported to date for this product and lot.Investigation summary: the physical device was not returned for evaluation. One radio graphic image and one photo were received and reviewed. The fluoroscopic image centered around the left arm shows a wire within the left arm and coursing within the chest.Based on the trajectory of the wire, it is likely traveling within the venous system.A stent is seen within the left chest, likely within the axillary vein. Also demonstrated in this image is an almost completely deflated angioplasty balloon. This appears to the left of the image near the patient¿s elbow. The provided images show an angioplasty balloon within the patient¿s arm, but further imaging or videos are needed to confirm the complaint. The photos show the distal end of the balloon and it appears bloody. The balloon was noted to be stuck with the introducer sheath. The distal end of the sheath was noted to be deformed. No other specific anomalies were noted from the provided photo. Therefore, the investigation remains inconclusive for the reported difficult to insert balloon through the sheath as no objective evidence could be noted from the provided radiographic image and submitted photo. However, the investigation was confirmed for the reported difficulty in removing the balloon from the sheath as the balloon was noted to be stuck within the introducer sheath which was observed from the provided photo.A definitive root cause for the reported difficult to insert , device-device incompatibility and difficult to remove could not be determined based upon the provided information.Labeling review: a review of product labeling documentation (e.G., procedural instructions, indications, warnings, precautions, cautions, possible complications, contraindications, nursing guide, and unit label) did not find any product labeling inadequacy.H10: b5, d4 (expiration date: 06/2024), g3, h6(device, method).H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
