Additional Manufacturer Narrative
H10: manufacturing review: a manufacturing review was not required as this is the only complaint reported to date for this product and lot.Investigation summary: one atlas gold pta dilatation catheter in two segments was returned for evaluation. One segment was noted to have a fragmented balloon piece, and another segment consisted of the remaining portion of the catheter and exposed inner guide wire lumen. All the anomalies are noted on microscopic observation. No functional testing was performed due to the condition of the sample and the nature of the complaint. Therefore, the investigation remains inconclusive for the reported balloon rupture as the returned balloon was disintegrated into multiple fragments and returned for evaluation. The investigation also remains inconclusive regarding the reported entrapment as the condition of use could not be replicated under laboratory conditions. However, the investigation is confirmed for the reported balloon detachment as the balloon was noted to have completely detached from the catheter and disintegrated into multiple fragments.A definitive root cause for the reported balloon rupture, entrapment and balloon detachment could not be determined based upon the provided information.Labeling review: a review of product labeling documentation (e.G., procedural instructions, indications, warnings, precautions, cautions, possible complications, contraindications, nursing guide, and unit label) did not find any product labeling inadequacy.H10: d4 (expiration date: 10/2024).H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
