Event Description
The information was received from health care provider via distributor regarding a patient with cervical discectomy for cervical distraction spinal therapy at l4-l5.It was reported that the distractor was broken into two parts.There was fragment of the implant or instrument remaining in the patient.There was no further information reported.
 
Manufacturer Narrative
