Event Description
Information was received from a patient who was implanted with an implantable neurostimulator (ins) for bowel dysfunction/sacral nerv stim.It was reported that the first implant only worked a little in the beginning however it was less than 50% improvement.Patient said that they made an adjustments however that didn't help.Patient said that later went to see hcp and was told that it wasn't working, doesn't recall any detailed reason why wasn't working however said that hcp replaced the system on (b)(6) 2021.

Product complaint #(b)(4).Investigation summary: the device associated with this report was received for analysis.Visual examination of the returned device confirmed the allegation.The ceramic insert rim was fractured in it entirety.Fragments of the device were returned however a several portion of the rim was not returned.Depuy considers the investigation closed.Should additional information be received, the information will be reviewed, and the investigation will be re-opened as necessary.Device history lot: a manufacturing records evaluation (mre) was not performed as no lot number was provided for this device.
