Event or Problem Description
It was reported that the apg+ curved handle did not hold sizer pin guides during case.Also sizer pin guides 52+0 was worn down and would not stay on the handle.All instruments were disposed of before being able to return back.All pieces were recovered.No surgical delay.

Product complaint # (b)(4).Investigation summary no device associated with this complaint was returned.Based on the photo evidence provided, it was identified that the post was broken off from the piece.The worn condition cannot be determine with the information provided.Additionally, the unable to assemble condition reported cannot be established due no physical device was returned.No other issues were identified.Depuy synthes considers the investigation closed at this time.Should additional information be received, the information will be reviewed and the investigation may be re-opened as necessary.Device history lot the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.
