Event Description

Information was received from a patient who was implanted with an implantable neurostimulator (ins) for spinal pain indications. The reason for call was 'settings not available'.Pt reported since implanted pt had had a lot of issues with therapy not working.Pt had seen a manufacturing representative (rep) a lot and last time rep set it on a.Pt said they were getting maybe 20% relief which was better than nothing.About a week ago pt started getting 'settings not available'.Pt said at first the device did not seem to want to recognize the ins.Pt was having to hold it right no top of ins before it would recognize it and let pt charge it.Pt confirmed ins was charged now but 'settings not available' was not resolved.Pt confirmed they had no falls/no trauma.Instructed pt to try other groups.Pt used the controller and had only group a.Additional information was reported that a manufacturing representative (rep) called in today and was with the pt.The caller noted she ran a 'connectivity check' and all contacts were red.The caller went on to run an impedance check and noted that all contacts are showing 'red' and >40k ohms. The caller states that the pt has been swimming a lot but noted there has been no falls/trauma.Technical services (tss) reviewed this is the reason for the 'settings not available' and reviewed that considering there are no viable impedance pairs the pt is likely not going to have therapy.

Additional information was received from the manufacturer representative. It was reported that they opened the ins pocket and tested the lead with a wens and most of the contacts were high impedance.Rep said the dr wanted to change the ins, so the dr used another ins and connected it to the lead and most of the contacts were still high impedance.Rep reports the dr is going to replace the lead.Rep called technical services (ts) after the dr replaced the ins.Ts reviewed that testing with the wens indicated the issue is with the lead.
