Event or Problem Description
Supplemental report is being submitted due to the completion of the investigation.User error was confirmed on 08-dec-2022 'as per the additional information received, when asked if the distal end of the stability sheath was inside the access sheath, the reply was no.'.

Additional Manufacturer Narrative
Pma/510(k) # p100022/s014 device evaluation the zisv6-35-125-6-100-ptx device of lot number c1938928 involved in this complaint was implanted in the patient and was not available for evaluation. With the information provided, a document-based investigation was conducted.Lab evaluation the device lab evaluation could not be completed as the device, or photographic evidence of the device, was not returned for evaluation.Document review prior to distribution all devices are subjected to a visual inspection and functional inspection to ensure device integrity.A review of the manufacturing records did not reveal any discrepancies that could have contributed to this complaint issue. The review of relevant manufacturing records confirms the failure mode has not previously occurred for this work order. It should be noted that the instructions for use (ifu0118) states the following: ¿note: ensure the distal end of the stability sheath is inside the introducer sheath¿.There is evidence to suggest that the customer did not follow the instructions for use.As per the additional information received, when asked if the distal end of the stability sheath was inside the access sheath, the reply was no.The user has not complied with the requirements of the ifu and/label.Image review an image was not returned for evaluation. It was originally stated that images were available but after numerous requests, these images have not been sent.Should the images become available at a later date, the file will be reopened and updated accordingly.Root cause review a definitive root cause was determined from the available information. The user did not comply with the requirements of the ifu and/label regarding the distal end of the stability sheath to be inside the access sheath. As per engineering input ¿sounds like the proximal end of the stent was angled with respect to the distal end because they talk below about straightening the stent. It also says below that the distal end of the stability sheath was not inside the access sheath. The ifu tells them to keep the distal end of stability sheath inside the introducer sheath (which is the access sheath) so this is a case of use error.User/use error related complaints are considered to be foreseen misuse. It is unknown how the device will perform outside of instructions for use.The user has not complied with the requirements of the ifu and/label.Summary the complaint is confirmed based on customer testimony.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence. An additional stent was placed to straighten and tack down the original stent during the same procedure.Complaints of this nature will continue to be monitored for potential emerging trends.
