Impact code f2301 captures the reportable event of retrieval of the detached forming mandrel tip using a forceps.Medical device problem code a0203 captures the reportable event of forming mandrel tip detached. The returned curving mandrel of the stonetome was analyzed, and a visual evaluation noted that the molding tip was detached from the curving mandrel and it was not returned.Per media analysis, the provided photo showed that the molding tip was detached from the curving mandrel. The curving mandrel was observed under magnification and no visual damage/problem found.Per the dimensional inspection, the thickness and the length of the coined end of the were measured and these were within specifications. No other problems with the device were noted. The reported event of molding tip was detached from the curving mandrel was confirmed.Upon analysis, it was found that the molding tip was detached from the curving mandrel which matches with the findings based on the media inspection; however, it was not returned.Based on the findings, the problem could have been generated due to handling/manipulation upon removal of the mandrel from the stonetome. Therefore, the most probable root cause of this complaint is adverse event related to procedure.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.
