Event Description
|
It was reported that the attempted left ventricular (lv) lead was difficult to move in the delivery catheter.After further inspection, the physician found insulation damage and an exposed conductor wire protruding from the tip of the lead.The lead was removed and replaced.No patient complications have been reported as a result of this event.
