This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda. Event Description
Information was received from multiple sources (manufacturer representative, healthcare provider) regarding a patient who was receiving fentanyl (150 mcg/ml at 16. 5 mcg/day) via an implantable pump for unknown indications for use. It was reported that the healthcare provider (hcp) tried to aspirate through the site port during the patient's scheduled refill in clinic and was not able to aspirate. There were no external factors involved. A revision occurred and the hcp was still unable to aspirate through the side port. They removed the catheter from the pump and cut the catheter next to the collet connector. They used a 24 gauge needle and were able to directly aspirate from the catheter. They then used an 8784 revision kit and placed a new proximal portion. After connecting to the pump they were able to aspirate through the side port. The event was resolved.
