Additional Manufacturer Narrative
This report is being supplemented to provide additional information based on the approved final investigation. The device history record was unable to be reviewed for this device since the serial number was not provided. However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.Based on the results of the investigation, the definitive root cause of the distal cover issue could not be determined. The lock mechanism of the cover did not have any problem. It is possible that there was an imperfection of the attachment method of the cover, which may have led to unfixing of the cover to the hook at the distal end of the scope, and the lock of the detachment prevention was not engaged.Olympus will continue to monitor field performance for this device.

This report is being supplemented to provide additional information based on the approved final investigation. The device history record was unable to be reviewed for this device since the serial number was unknown. However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.Based on the results of the investigation, and since no device malfunction was confirmed during evaluation, the definitive root cause of the distal cover detachment could not be determined. It is possible that imperfection of the attachment method of the cover may have led to the unfixing of the cover to the hook at the distal end of the scope, which did not engage the detachment prevention lock. The maj-311/maj 411 abridged instruction manual identifies verbiage which may have prevented the phenomenon.Olympus will continue to monitor field performance for this device.
