Event Description
|
It was reported that the cardiac resynchronization therapy defibrillator (crt-d) delivered an inappropriate shock due to right ventricular (rv) lead oversensing.It was also noted that oversensing and noise was observed on the rv lead which resulted in pauses in the ventricular rhythm.A x-ray confirmed that the rv lead pin was not deep seated into the crt-d header.A magnet was applied to the crt-d to suspend therapy and the patient will be monitored.It was further reported that the left ventricular (lv) lead exhibited an increasing capture threshold.The crt-d and the ventricular leads remain in use.No further patient complications have been reported as a result of this event.

Additional information received noted surgery was performed to remove the lead and re-insert into the device.The header was visualized on fluoroscopy to confirm proper insertion.
