Event or Problem Description
On (b)(6) 2022 it was reported by a sales representative via sems that an ar-13974nr fiber tape retriever broke during a shoulder scope.The tip broke off in the joint.The surgeon identified and retrieved the tip.The case was completed as usual with no patient affect.
