The returned instrument was subjected to a detailed technical analysis and the corresponding product release documentation was reviewed to establish whether a deviation from the manufacturing process could be the cause for the reported event. The technical investigation revealed that the inner shaft has fractured and separated from the rest of the system. The separated inner shaft fragment has a length of about 527 mm.The fragment is severely elongated and the fracture site is plastically deformed, both indicating application of a high tensile force. The stabilizer shaft shows severe compression marks, particularly at the transition to the handle lever. This finding implies that the retractable shaft has been pulled too far back after stent release. Moreover, the retractable shaft shows signs of compression in its distal portion, indicating application of a pushing force despite meeting resistance. The exact sequence of these events cannot be reconstructed. The introducer sheath and the guidewire used in the intervention were not returned for analysis.Review of the product release documentation confirmed that the device in question was manufactured according to specifications and successfully passed all in-process controls as well as the final inspection.Based on the conducted investigations, no material or manufacturing related root cause was determined. The root cause for the complaint event is most likely related to the handling during the procedure. Please note that the ifu advises the user to exercise care during handling to reduce the possibility of accidental breakage of the delivery system shaft.
