Device report from (b)(6) reports an event as follows: it was reported that on (b)(6) 2022, during percutaneous vertebroplasty (l3) for treating centrum fracture, it took 15 minutes for cement preparation.The cement was not easily injected with a 2mm syringe, so it was exchanged for a 1mm syringe.When the 2mm of cement was applied to each side, the cement became harder.As a result, a cement needle got stuck to the cement and became immovable.The surgeon decided to leave the needle in the lesion, the procedure was delayed by more than 30 minutes.No further information is available.This report is for an access kit-sterile.This is report 1 of 1 for (b)(4).

Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.H3, h6: part: 03.804.612s, lot: 22d25-2, manufacturing site: werk selzach, release to warehouse date: 25 april 2022, expiration date: 25 april 2027, supplier: (b)(4).A manufacturing record evaluation was performed for the finished article lot and no non-conformances were identified.The photo was returned to depuy synthes for evaluation.The depuy synthes team conducted a visual inspection of the device from the received image(s).Visual analysis of the post-operative images revealed that the cement needle remains at the implantation site.A functional test to assess the assembly issues was not conducted since the device was not returned.No other problems identified.The product was returned to depuy synthes for evaluation.The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the tip of the needle was broken.The broken fragment was not returned for evaluation.The embedded device condition can not be confirmed as no post-operative images were provided.A dimensional inspection was unable to be performed due to post-manufacturing damage.A functional test was not conducted since the device was returned in a disassembled condition and exhibited post-manufacturing damage.As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.The overall complaint was confirmed as the observed condition of the device would contribute to the complained device issue.There is no indication that a design or manufacturing issue has caused the complaint condition and hence the root cause cannot be determined.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Drawing/specifications reviewed current and manufactured revisions were reviewed.Device was used for treatment, not diagnosis.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
