Event or Problem Description

|
As reported, during an unspecified procedure involving the right femoral artery, a micropuncture transitionless stiffened cannula access set's wire unraveled and separated upon insertion, leaving a small fragment in the patient.Access was obtained in the femoral artery and the access site was reportedly calcified.Resistance was encountered upon both insertion and removal of the wire, which was manipulated and/or removed through the entry needle. The patient was transferred to another facility for removal of the wire fragment; however, the decision was made not to remove it.The fragment reportedly remains in the left groin, either extravascular to the femoral artery, or slightly intravascular.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.

Event description: as reported, during an unspecified procedure involving the right femoral artery, a micropuncture transitionless stiffened cannula access set's wire unraveled and separated upon insertion, leaving a small fragment in the patient.Access was obtained in the femoral artery and the access site was reportedly calcified.Resistance was encountered upon both insertion and removal of the wire, which was manipulated and/or removed through the entry needle. The patient was transferred to another facility for removal of the wire fragment; however, the decision was made not to remove it.The fragment reportedly remains in the extravascular right groin, outside of the right femoral artery.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.Investigation - evaluation reviews of complaint history, device history record (dhr), manufacturing instructions, instructions for use (ifu) and quality control procedures were conducted during the investigation. The complaint device was not returned; therefore, no physical examinations could be performed; however, a document based investigation evaluation was performed.A review of the device history record found no non-conformances related to the reported failure mode.A review of complaint history records shows no other related complaints associated with the complaint device lot.Cook concluded that no nonconforming product from this lot exists in house or in the field.A document review was completed as a response to this event.A device master record (dmr) review was performed, including device manufacturing instructions and quality control documents.Cook has concluded that sufficient inspection activities are in place to identify this failure mode prior to distribution and there is evidence that the device was manufactured to specification. The instructions for use (ifu), provides the following information to the user related to the reported failure mode: precautions ¿do not attempt to insert or withdraw the wire guide and/or introducer if resistance is felt.¿ ¿withdrawal or manipulation of the distal spring coil portion of the mandril wire guide through a needle tip may result in breakage.¿ instructions for use ¿advance the.018-inch wire guide through the introducer needle.Caution: do not withdraw the wire guide through the introducer needle; breakage my result. If the wire guide tip must be withdrawn while the needle is inserted, remove both the needle and the wire as a unit.¿ cook has concluded patient anatomy contributed to this incident.Resistance was felt during insertion and removal due to the noted calcified anatomy per the quality engineering risk assessment, no further action is required. The appropriate personnel have been notified, and we will continue to monitor for similar complaints. This report is required by the fda under 21 cfr part 803.This report is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement made in it is intended to be an admission that any cook device is defective or malfunctioned; that a death or serious injury occurred; or that any cook device caused or contributed to; or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
