According to the reporter, the product was a known recalled product but per regionally communicated email from the senior clinical risk advisor from alberta health services, the device could be used under the discretion of the physician. It was decided that it was the appropriate catheter to be used for hemodialysis (hd) central line insertion and so to ensure functionality of the medical device proper direction was followed. It was stated that the physician, technologist and nurse triple checked the device. It was noted that upon infusion of saline into the venous port, reflux of fluid was coming back into the opposite arterial port. This was observed through the clear portion of the catheter, distal to the endcaps. The catheter was removed from the sterile field.A second catheter was opened, and triple checked and was found to be in proper operation. It was documented in the chart that the catheter was verified safe to use by the radiologist. There were no patient symptoms or complications associated with this event. There was no reported patient injury.
