The lens remains implanted. The foreign material was not returned.A video was provided. The lens/cartridge preparation and lens delivery were not shown. The video started with the lens already implanted. The lens was a multifocal toric single-piece lens.A possible mark or material was faintly visible. The lens position did not allow for a better view. The camera shifted off the eye. When the view was placed back on the eye, no foreign material was observed. The camera partially shifted off the eye again.Only a corner of the lens was visible as it was aligned. The lens remained implanted.Product history records were reviewed and documentation indicated the product met release criteria.A qualified cartridge was indicated. The handpiece and viscoelastic used were not provided. It is unknown if qualified products were used. The product investigation could not identify a root cause for the reported complaint. The nature and origin of the foreign material cannot be determined from the video. No determination can be made without physical evaluation of the complaint sample. It is unknown if a qualified handpiece and viscoelastic were used.Per the instruction for use (ifu): company foldable iols are qualified for use with an company qualified delivery system (handpiece and cartridge) and ophthalmic viscosurgical device (ovd) combination. The use of an unqualified combination may cause damage to the iol and potential complications during the implantation process. The ifu also instructs to completely fill the cartridge with ovd immediately prior to loading and delivery of the lens.Do not attempt to load the lens without adequate ovd in the device. Not adequately filling the device with viscoelastic will result in inadequate coverage of lens and the lens fold path with ovd, which may result in damage. There are no other complaints in this lot.The manufacturer internal reference number is: (b)(4).
