Event Description
|
It was reported that the cardiac resynchronization therapy defibrillator (crt-d) exhibited a full reset, and the device settings were automatically adjusted back to nominal settings.It was also noted that inappropriate internal programming of a wavelet template parameter occurred.When the patient was brought to the clinic to have the device programmed back to the original settings an x-ray was performed and it was found that all the leads had pulled back.The right atrial (ra) lead exhibited high thresholds and was not capturing at high outputs.The x-ray confirmed the lead had dislodged.A revision was attempted but too difficult to maneuver so the ra lead was explanted and replaced.The right ventricular (rv) lead trends were stable but due to having pulled back slightly it was repositioned.The left ventricular (lv) lead also exhibited high thresholds and it was suspected to have slightly pulled back.The lead was attempted to be repositioned.When this attempt to reposition failed, two different lv lead were attempted but could not be placed due to patient anatomy.Due to the inability to place a new lv lead the device was downgraded to an implantable cardioverter defibrillator (icd).The device was originally reprogrammed and later explanted and replaced due to downgrade.The rv lead remains in use.No patient complications have been reported as a result of this event.
