Additional Manufacturer Narrative
Additional information: an attempt was made to use one resolute onyx coronary drug eluting stent to treat a lesion with 90% ostial stenosis in the second obtuse marginal branch. The device was inspected before use with no issues noted. The device was prepped per ifu with no issues noted. It was initially attempted to advance 2 mm x 15 mm non mdt balloon into the obtuse marginal branch but this was unsuccessful.Instead, 1.5 mm x 15 mm non mdt balloon was advanced and multiple inflations were performed at 10 to 14 atm.Initially the ptca result appeared to be adequate.Subsequent angiographic views demonstrated substantial recoil in the ostium of the second obtuse marginal branch with questionable dissection. Therefore, a decision was made to proceed with stenting of the obtuse marginal branch.A 2 mm x 15 mm resolute onyx drug-eluting stent was introduced over the guidewire. The attempt to advance the stent into the ostium of the obtuse marginal branch was unsuccessful.During the withdrawal of the stent into the guide catheter, it was sheared inside the previously deployed stent in the proximal lcx.An attempt was first made to retrieve the stent using the snare, however this was unsuccessful due to inability to advance the snare beyond the proximal lcx stenosis.A second 300 cm non mdt wire was introduced next to the stent.Multiple inflations were performed in an attempt to crush the stent inside the previously deployed stent but it was noted that the stent remained to be mobile. Therefore a decision was made to advance a second stent next to the last stent which was accomplished using 2.5 mm x 15 mm resolute onyx drug-eluting stent which was deployed at 20 atm.The second obtuse marginal branch was predilated using 1.5 mm x 15 mm non mdt balloon at 12 atm. This was followed by 2 mm x 15 mm non mdt balloon at 16 atm.Final angiographic views demonstrated 0% residual stenosis in the proximal lcx.The stent that was sheared was completely excluded by the second deployed stent. There was minimal residual stenosis in the ostium of the second obtuse marginal branch and distal flow was timi grade iii.Patient history and weight provided.Event date provided.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms. This statement should be included with any information or report disclosed to the public under the freedom of information act.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report. If information is provided in the future, a supplemental report will be issued.
