Event Description
The following was reported to gore: in (b)(6) 2019, the patient underwent dialysis shunt construction using gore® acuseal vascular graft for renal failure.On (b)(6) 2022, tear and hematoma were observed in inner layer and middle layer, and delamination of the device was suspected.On (b)(6) 2022, the torn part of the device was explanted and replaced with a new graft.The patient tolerated the procedure.
 
Manufacturer Narrative
Product history records: a review of the manufacturing history of the product could not be conducted as the serial/lot number remains unknown.Engineering evaluation: returned were four gore® acuseal vascular graft fragments submitted in formalin that had been transected prior to arrival at w.L.Gore and associates.There were multifocal regions of layer separation between the middle and outer layers of the returned graft specimens with thin layers of tissue deposition present indicative of separation in situ.The inner (luminal) eptfe layer remained firmly adhered to the middle layer except where small tags had been disrupted by backwall needle cannulation.There were multiple instances of shallow angle needle cannulation, fissures and needle drag indicative of suboptimal cannulation technique which results in damage to the graft.The exact cause of layer separation could not be determined.
