Event or Problem Description
It was reported a new trach was put on the patient when the parent noticed that the patient started to desat. The patient then had to be bagged with a resus bag. They removed the trach and saw that it was not fully inflated. The patient's mother then tested the backup trach they had and found it would not fully inflate either so mom put the old trach back in the patient. The mother did not want to do this as the old trach has mold in it.No patient injury reported.
