Event or Problem Description
It was reported that prior to use of the 2.50x15mm xience skypoint stent delivery system (sds) the protective sheath was removed without resistance. The catheter was soaked in saline and the device was prepped (air aspiration) inside the anatomy. The procedure was to treat a 90% stenosed lesion in the distal left circumflex artery with heavy calcification and heavy tortuosity. The sds was advanced to the target lesion with difficulty due to the anatomy. However, the balloon burst during the first inflation after 20 seconds at 12 atmospheres (atm). The sds was removed from the patient. There was no adverse patient effect and no clinically significant delay reported in the procedure. No additional information has been provided.

Additional Manufacturer Narrative
The device was not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar incidents from this lot.It was reported xience sypoint stent delivery system (sds) was prepped inside the anatomy. It should be noted that the xience skypoint everolimus eluting coronary stent system instruction for use (ifu) lists delivery system preparation to be performed prior to delivery procedure. In this case, it is unknown if the instruction for use (ifu) deviation related to incorrect stent preparation may have contributed to the reported event. The investigation determined the reported difficulty to advance and material deformation appear to be related to circumstances of the procedure. The investigation was unable to determine a conclusive cause for the reported material rupture. There is no indication of a product quality issue with respect to manufacture, design or labeling.
