Event or Problem Description
It was reported that the procedure was to treat a lesion in an unspecified artery. The 3.0x120mm armada 14 balloon dilatation catheter (bdc) was positioned and as the physician wanted to change the guidewire, the hi-torque (ht) command 14 guidewire went into the wrong balloon lumen [tear to armada 14 balloon].The armada 14 balloon and ht command 14 guidewire had to be removed.A new armada and guidewire were used to complete the procedure. There was no adverse patient effect or clinically significant delay in the procedure. No additional information was provided.

Additional Manufacturer Narrative
Visual and scanning electron microscopy (sem) analysis was performed on the returned device. The reported tear/hole in the inner member within the balloon was confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar complaints reported from this lot.The investigation was unable to determine a definitive cause for the reported difficulties. However, based on the reported information and evaluation of the returned product, it may be possible that while backloading the armada 14 over the first guide wire, the proximal end of the wire may have inadvertently punctured the inner lumen within the balloon.Additionally, when the guide wire was exchanged for the ht command, as the wire was being advanced through the proximal end of the catheter, the tip of the wire found its way into the existing puncture and ended up in the balloon. The knot observed in the wire tip may have occurred due to the wire curling up on itself within the balloon creating a knot. However, this could not be confirmed. There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
