Medtronic received information that an axium coil had resistance in the echelon and the coil had been separated/broken/prematurely detached. After the microcatheter was in place, the surgeon took out the apb-5-15-3d coil for normal hydration in vitro. When entering the starting position of the microcatheter through the y valve, it was found that there was resistance and could not be pushed forward. When withdrawing, it was found that the spring coil had been stretched and wire drawn. The operator believed that this was a product quality problem, and requested to return it, please identify it.No surgical or medical intervention was required. The pushwire was not bent or broken. The physician did not reposition the coil. The physician did not attempt to detach the coil. The physician did not rotate the delivery pusher during the procedure.Continuous flush was administered during the procedure. The devices were prepared as indicated in the instructions for use (ifu). The patient was being treated for an unruptured, amorphous aneurysm in the basilar artery. The max diameter was 6.5mm and the neck diameter was 4.3mm.Patient blood flow was normal and vessel tortuosity was moderate. No patient injury or symptoms were reported.

Product analysis: as found condition: the axium prime coil was returned for analysis within a shipping box, within a plastic bio-pouch, and within a dispenser coil.Visual inspection/damage location details: the axium prime coil was returned within its introducer sheath. The introducer sheath was found inverted on the axium prime coil with the implant coil within the wavelock section. No damages were found with the introducer sheath. The implant coil was found damaged within the introducer sheath. The axium prime coil was pulled out proximally from within the introducer sheath for further analysis. No bends or kinks were found with the pusher. The implant coil was found still attached to the pusher. The implant coil was found damage and had lost its original shape.Upon further examination, the implant coil distal tip appears to have been cut.Testing/analysis: the axium prime coil was pulled into the introducer sheath. The implant coil length was measured to be ~15.0cm.Conclusion: based on the device analysis and reported information, the customer¿s ¿coil resistance/stuck in sheath¿ and ¿coil resistance/stuck at cath.Hub¿ reports were confirmed.Possible causes of ¿coil resistance/stuck at cath.Hub¿ include use of an incompatible catheter, damage to coil due to excessive tightening of rhv, introducer sheath damaged, introducer sheath not properly seated in hub, or catheter hub transition.Possible causes of ¿coil resistance/stuck in sheath¿ include damage during preparation, overtightening of rhv, improper hubbing technique, blood in sheath due to lack of continuous flush, or inverted introducer sheath.Improper hubbing technique could not be ruled out as a potential cause. The echelon micro catheter is compatible for use with the axium prime coil. The echelon micro catheter used in the event was not returned. Therefore, an analysis could not be performed, and any contributing factors could not be assessed. In this event, it is likely the damages found with the returned axium prime implant coil contributed to the reported resistance. However, the cause for the damages could not be determined.Regarding the customer¿s ¿coil stretch¿ and ¿premature detachment¿ reports the issues could not be confirmed. No stretching was found with the axium prime coil and the implant coil was found still attached to the pusher.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms. This statement should be included with any information or report disclosed to the public under the freedom of information act.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report. If information is provided in the future, a supplemental report will be issued.
