It was reported that the procedure was to treat a heavily tortuous, moderately calcified right common iliac artery. The 9.0x19mm omnilink elite delivery system was noted to have met resistance with anatomy during advancement and removal of the device.Once removed it was noted that the stent dislodged on the delivery system. Not completely off but not well placed.Another same size ominilink was used to complete the procedure. There were no adverse patient effects and no clinically significant delay. No additional information was provided.

Additional Manufacturer Narrative

A visual inspection was performed on the returned device. The reported stent dislodgement was confirmed. The reported difficult to advance and difficult to remove could not be evaluated as the exact anatomical conditions encountered by the device used during the procedure could not be replicated in the test laboratory.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar incidents from this lot.The investigation determined the reported difficulties appear to be related to circumstances of the procedure. There is no indication of a product quality issue with respect to manufacture, design, or labeling.
