Event Description
According to the reporter, during a open repair pre-peritoneal umbilical hernia procedure, after being hydrated, the device appeared as if the barrier had dissolved.

Event Description
According to the reporter, during an open repair pre-peritoneal umbilical hernia procedure, after being hydrated, the device appeared as if the barrier had dissolved.The mesh was not used on the patient.To resolve the issue, a new device was used.There was no patient injury.
